Please arrive by 4pm - Since these are drop-in events, we cannot predict wait times
Ahead of time you will need to prepare the following:
ID Documents: Bring OR take clear pictures of your Birth Certificate & Government Issued ID (even if you don't have these you can still come to start the process!)
Proof of Residency (if applicable): If you DO NOT have an Illinois ID or Driver’s License, please bring proof of residency in Illinois and in Cook County. You must be a resident in Illinois for 6 months to be eligible for a name change.
Criminal Convictions: If you have any criminal convictions, please provide any information you may have regarding the conviction, including: where the conviction occurred (i.e., county and state), case number if you know it, sentence, and name of crime(s) (i.e., possession of a controlled substance, robbery, retail theft, etc).
Intake: Fill out our online intake form (you can also fill it out on your phone when you arrive).
Fee Waiver: If you would like to apply for a waiver of court fees, be prepared to discuss your income in the past 30 days and in the past 12 months (bring proof of income if possible - Examples include: Proof of SNAP and/or SSI, W2, 1099, pay stubs, etc.)
